One of the problems I've run into in the past, is that the people who make up those entry forms (prospectuses) do NOT always follow a 'library standard,' nor any standard often as not. They usually tell you how THEY want the slides labeled and where to put the red dot (or an 'up' arrow).
I use my software programs to design slide label forms that allow me to flexibly change the layout to suit the individual requirements of the contest or gallery requesting the slides. I print out onto Avery labels with an outline of the 35mm slide as a cutting guide that allows me to trim the generic labels to size. I don't bother with finding labels made for slides since these are hard. I use the labels that come five to a letter-size sheet.
NOTE: Once the label is attached to the slide mount, be sure and trim any excess with a sharp craft knife or straight razor blade. You don't want any of the self-stick glue on the label cause slides to stick together or jam a projector.
I never send off slides in cardboard mounts. I always remount the film in stiffer plastic mounts. I buy the type that have a 'snap-lock' feature that makes them very simple to work with.
